Key responsibilities
- Deliver high quality care to patients with complex physical health care needs in their homes or designated settings.
- Manage tracheostomies, ventilators, enteral feeding, complex seizures, and medication administration.
- Assess patient care needs daily, implementing and evaluating care plans produced by qualified nurses.
- Regularly assess care plans, risk assessments, and contingencies with qualified nurses to ensure consistency and up-to-date care.
- Ensure care delivery complies with local and Trust agreed policies and procedures and maintains the highest standard.
- Assist patients in achieving their potential to live independently as per their care plan.
Essential criteria
- NVQ 3 or Nursery Nurse qualification or equivalent
- Significant experience in a similar role
- Please demonstrate within your application significant work experience of dealing with complex care needs, including management of tracheostomies, ventilators, enteral feeding, complex seizures and medication administration.

Job summary
In the Community Complex Care Team pride ourselves on our delivery of excellent, holistic patient and family centred care within the home, school and a variety of community settings. All of our client group (Adults and Children) have highly complex physical health needs, from needing support with their breathing to managing challenging seizure activity and everything in between!
We are a large, expanding team covering the whole of Staffordshire, with office bases in Stafford, Stoke-On-Trent and Lichfield. We operate in a Hub system, which is, based on geographical location allowing us to ensure a supportive, small team feel across all hubs. We provide a 24/7 nurse on call service, ensuring a constant point of contact where support can be accessed.
We are looking for Complex Care Support Workers to join our team. We do cover a 24-hour period so there is an expectation that you would be able to work days and nights and the shift times are individual to each package of care. The whole of your shift will be spent with one client, so you will get the opportunity to really make a difference.
During interview you will be able discuss which geographical location you would be able tocover. You must be able to travel to areas and for shift times where public transport is not available.
