Role summary
This role is for a Band 6 Community Sister/Charge Nurse to join the Virtual Ward team, providing high-quality, person-centred nursing care to patients in their homes. You will manage a defined caseload, provide clinical leadership to junior staff, and undertake comprehensive patient assessments and care planning. The service operates 7 days a week from 8:00am to 8:00pm. A full UK driving licence and access to a car are essential.
Key responsibilities
- Deliver high-quality, person-centred nursing care to patients within the Virtual Ward.
- Work autonomously, managing a defined caseload of patients.
- Provide clinical leadership, supervision, and support to junior staff.
- Undertake comprehensive and holistic patient assessments, develop and implement care plans, and evaluate patient outcomes.
- Collaborate closely with Frailty Consultant Physicians, participating in daily virtual ward rounds and contributing to patient treatment and care planning.
- Communicate effectively with family, carers, the multidisciplinary team, and other agencies to facilitate timely discharge and maintain patient independence at home.
Essential criteria
- Experience of working post registration in a wide range of clinical environments
- Experience of MDT working
- Experience in line managing junior staff
- Experience in working in a community healthcare setting
- Experience of initiating and participating in clinical audit
- Awareness of current healthcare legislation affecting community and secondary care services
- Clear understanding of health and social care services
- Competency an ability to assess and treat a wide range of complex conditions within related field
- Excellent communication and interpersonal skills
- Decision making, prioritising and problem solving skills
- Ability to organise the workload, able to delegate and prioritise
- Good knowledge of health and safety and risk management
- Basic IT skills
- Flexible approach to service delivery
- Ability to work as part of a multidisciplinary team
- Demonstrates an awareness of the complex issues involved when working across organisations and disciplines
- Relevant NMC qualification

leadership of the Virtual Ward Team Lead.
As part of
the Responsive Services team, you will also work collaboratively within the
wider Urgent Community Response and Virtual Ward teams.
Aligned with
the NHS Virtual Ward model, our service enables patients to receive safe,
effective, and high-quality care in the comfort of their own homes as an
alternative to hospital admission. Using a combination of clinical expertise
and digital technology, we support acutely unwell patients to be cared for
safely at home while maintaining close clinical oversight.
This is an
excellent opportunity for a motivated and compassionate nurse who is passionate
about delivering innovative, patient-centred care. You will work as part of a
multidisciplinary team to provide outstanding care to patients and their
families, ensuring a seamless and responsive service.
The Virtual
Ward operates 7 days a week from 8:00am to 8:00pm, offering continuity of care
