This role is for a passionate Specialist Diabetes Practitioner to join the Northern Care Alliance, delivering high-quality, evidence-based diabetes care. The position involves cross-site cover at Royal Oldham Hospital and Fairfield General Hospital, focusing on improving patient outcomes through education and innovative diabetes management.
Key responsibilities
- Deliver a high-quality, evidence-based service for people living with diabetes.
- Assess, plan, implement, and evaluate individualized treatment plans.
- Act as a specialist resource, providing expert advice to patients, families, and healthcare professionals.
- Deliver education and training to staff and students on all aspects of diabetes management.
- Participate in research and clinical audits.
- Promote excellent diabetes care to reduce inpatient bed occupancy and empower patients.
Essential criteria
- Substantial years experience post registration
- Registered Nurse with current NMC registration (Part 1 Adult / General Level 1 or 2)
- Degree / Diploma in Nursing studies or equivalent
- Recognised teaching qualification
- Experience of working in the field of diabetes and good clinical knowledge
- Evidence of proactive involvement in the mentorship of nurses
- Excellent interpersonal skills, evidence of development of own knowledge
- Ability to travel to different geographical locations across the Trust footprint in line with service needs
Desirable criteria
- Non-medical prescriber or willingness to undertake
- Post basic qualification in diabetes or equivalent portfolio of evidence
- Awareness of diabetes technology
- Full UK driving license with willingness to use own car for work purposes
