Role summary
The Transplant Specialist Nurse will provide expert clinical leadership and specialist nursing care for patients undergoing kidney transplantation. This role involves leading the development of nurse-led services, collaborating with multidisciplinary teams to improve patient outcomes, and promoting best practice throughout the transplant pathway. The post holder will manage a defined caseload, deliver outpatient clinics, and act as an expert practitioner and role model.
Key responsibilities
- Lead and develop a nurse-led transplant service, providing specialist care and support to renal transplant patients throughout their transplant journey.
- Assess, plan, implement and evaluate complex care for patients within a variety of clinical settings, ensuring care is evidence-based and individually tailored.
- Manage and coordinate a defined caseload of transplant patients, facilitating continuity of care and effective follow-up.
- Lead the development, implementation and review of transplant care pathways, protocols, guidelines and clinical standards.
- Deliver nurse-led outpatient clinics in partnership with medical colleagues, contributing to the ongoing monitoring and management of transplant recipients.
- Provide specialist education, training and mentorship to healthcare professionals, students, patients and carers.
Essential criteria
- BSc Hons in Adult Nursing
- Active professional registration qualification with professional body NMC
- Holds PACRR, PADRAP and Non -medical prescribing
- Evidence of ongoing professional development
- working towards MSc in Nurse health care
- Evidence of ongoing professional development relevant to transplant care/ Renal care/or similar
- Excellent organisational skills
- Renal and transplant MSc or similar experience
Desirable criteria
- demonstrate ability to question and challenge practice in a constructive way
- Experience of implementing local/specialist objectives
- Ability to act independently, resolve complex clinical situations and have good decision-making skills
