Role summary
This role is for a Personalisation and Personal Health Budget (PHB) Practitioner within the Hampshire and IOW All Age Continuing Care Personalisation service. Essential criteria
- Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area
- GCSE or equivalent qualification in mathematics, grade A-C
- Full UK Driving Licence and access to a car

- Mathematics A-Level of equivalent
- Qualifications in computing e.g. MS Excel, databases
- Knowledge of administrative procedures, project management or information analysis
- Experience of managing budgets and financial accounts
- Skills for communication of complex information and administrative matters, requiring developed interpersonal and oral/ written / presentation communication skills
- Ability to pull together comprehensive reports, data and letters
- Experience and understanding of Personal Health Budgets, Direct Payments or Self Directed Support, including personalised care support planning, budget setting, recruiting and employing Personal Assistants in Care (PAs)
- Basic knowledge of project principles and contract management
- Previously worked in similar position within the public sector

- Skills for nurturing key relationships and maintaining networks
- Ability to make recommendations on difficult and contentious issues and escalate them

- Excellent time management skills with the ability to re-prioritise
- Ability to travel between locations not accessible via public transport

- Elements of sustained computer working

We have a new opportunity within our Hampshire and IOW All Age Continuing Care Personalisation service. We are looking for a person who is passionate about client care and improving life opportunities who share our AACC behaviours.
As a Personalisation and Personal Health Budget (PHB) Practitioner, you will be responsible for maximising the take-up of PHBs and acting as a source of expertise and advice for members of the multi-disciplinary teams, patients and members of the public in order to support the PHB Process. The role involves working in partnership with clinicians to implement PHBs, including supporting and advising on the development of care and support plans, monitoring and managing client financial information and payments and providing advice and guidance to clients to empower people with complex health needs live their best lives.
Experience in Personal Health Budgets and accounting are essential, and we can support you to further develop any specific skills required. You will be part of a wider personalisation and All Age team who provide a wide range of expertise and support.
