Role summary
This role is for a motivated and experienced Mental Health Nurse to join the Restormel Community Mental Health Team. Key responsibilities
- Lead the duty desk service, managing incoming telephone calls and triaging referrals.
- Coordinate information and work closely with colleagues across the multidisciplinary team.
- Be responsible for the assessment of care needs and clinical management of a defined caseload.
- Carry continuing responsibility for the development, implementation and evaluation of programs of care, utilising the care co-ordination system.
- Develop the interface between primary and secondary care services to offer focused and co-ordinated packages of care.
Essential criteria
- Current professional registration - NMC
- Professional qualification - RMN
- Post graduate qualification or relevant experience
- Extensive experience of working with mental health patients

- Knowledge around suicide awareness and prevention and the ability to support patients
