An exciting opportunity has arisen to join the South Skye Community nursing team. It is expected that the HCSW will work independently with patients in the community and within the agreed parameters of a defined care plan. This may be in an individual’s home, community hospital or any other appropriate setting.
They will work as part of the multidisciplinary team, enabling people to achieve their optimal level of independence utilising a reablement/enablement programme of care.
They will assist in providing nursing care for those who are experiencing short term acute illness, exacerbation of long-term conditions or those wishing to receive end of life
