Role summary
This is a permanent, full-time Community Staff Nurse position within the District Nursing Service in Stockport. The role involves providing patient care in homes and residential settings, supporting individuals with complex needs and their carers. The service operates seven days a week, aiming to deliver timely and appropriate care to help residents maintain health and independence. The successful candidate will work with multidisciplinary teams and Mastercall GPs.
Essential criteria
- Occupational Health Clearance
- to travel independently and efficiently across multiple community sites to meet the operational requirements of the role
- Mental effort to exert frequent concentration where work pattern is unpredictable.
Desirable criteria
- Emotional effort. The post holder will be exposed to frequent distressing and occasionally highly distressing and emotional circumstances
- May be exposed to highly unpleasant working condition/some exposure to hazards.
- Delivery of patient care and working in a clinical area.
- Evidence of teaching and assessing students and junior members of staff
- Developing areas of expertise in community related practice. Tissue Viability/ Frailty/ end of life care
- Experience of audit involvement in project work.
- Current 1st Level Registration with NMC
- Grade C/4 (or equivalent) in GCSE English
- Teaching and Assessing Certificate
- Evidence of Health-related study at level 6/7
- Diploma in Nursing Practice or degree

Main duties of the job
The post holders will work with the Community Nurses and other multidisciplinary teams with the support of Mastercall GPs, to provide a first class service to the residents of Stockport.

The post holder should have good organisational and assessment skills be flexible and willing to cover the service between 08:00 - 18:00 over a 7 day period.
