Role summary
This role is for a registered nurse to join the District Nurses in Powys, providing autonomous nursing care to patients in their own homes within a rural setting. The post holder will deliver skilled care as part of a multi-disciplinary team, focusing on palliative care, end-of-life support, and managing long-term health conditions. It involves promoting self-management skills and wound care. Applicants who do not meet Band 6 competencies may be appointed under Annex 21 rules.
Essential criteria
- NMC Registered Nurse
- Post registration District Nursing qualification (BSc in relevant subject)
- Expeience of proactive caseload management of wide range of patients including those with chronic conditions
- For Band 6 Annex 21: to already be undertaking the 2 year qualification for DN SPQ
Desirable criteria
- Independent prescribing qualification
- MSc Advanced clinical Practice
- Management/leadership qualification or equivalent preparation
- Experience at management level demonstrating ability to deliver improvements to service users care
- Significant clinical experience in assessing, planning, implementing and evaluating cae for the patient group of the caseloadExperience in a wide range of clinical skills
- Competence and confidence in a wide range of clinical skills including palliative care, chronic disease management and wound management
- Aptitude and Abilities
- A high level of interpersonal and communication skills
- Standard keyboard skills and IT literacy
- Ability to engage with members of the public
- Ability to work independently and as part of a team
- Ability to speak or learn Welsh to a satisfactory level
- Local knowledge of statutory and voluntary services
- Other
- Ability to diffuse difficult/threatening situations
- Ability to deal with complex/stressful situations
- Able to work in a multi/disciplinary/multi-agency arena
- Excellent organisational skills

Main duties of the job
