Job summary
The Patient Safety Incident Response Framework (PSIRF) has now been implemented within Sandwell and West Birmingham Hospitals NHS Trust (SWBH) for two years and our Patient Safety Investigators play a key role in the continued embedding and development of our processes. We are looking to recruit an enthusiastic individual with a passion for improving the safety and experience of Our Patients, People and Population to join our team and help us continue to learn and improve.
As a Patient Safety Investigator within the Corporate Patient Safety Team, you will work closely with the Clinical Divisions to promote a culture of openness and learning from good practice and areas that may need some improvement. You will lead on a range of incident investigations looking at systems and processes using the SEIPS tools and support clinical staff to understand and use a range of learning tools including swarm huddles, After Action Review and Multidisciplinary Team reviews.
Supported by the Quality Improvement team you will be part of a team to recommend and implement improvements across the Trust, working across a wide range of teams. You will also engage with patients and their families to understand their perspective as part of your investigations, ensuring their experience is at the centre of all improvements made.
