Role summary
This is an exciting new permanent role for a Discharge Liaison Officer within the MAIDT team. The post holder will support and facilitate the safe and timely discharge of patients by communicating with BTHFT colleagues, the MDT, patients, families, and medical/nursing staff. The role involves ensuring everyone is working to the same discharge plan and supporting effective discharge processes.

Essential criteria
- Educated to GCSE English/Maths
- Experience of working as part of a team
- Experience of working in an Acute Trust
- Experience of working in the NHS
- Experience, knowledge and awareness of patient discharges
- Experience of dealing with members of the public
- Basic IT skills
- Good verbal and written command of English
- Good interpersonal skills
- Numeracy skills (demonstrated through qualifications or experience
- Ability to plan/organise own and delegated workload and work under pressure in a busy environment in a professional manner
- Aware of own strengths and limitations/levels of competence
- Record keeping and documentation
- Ability to respond to changes in patient condition
- Ability to work as a team member
- Motivation, reliability and ability to work as part of a team
- Flexibility and adaptability
- Understanding of Information Governance and Confidentiality
- Understanding of equality and diversity issues and how this affects patients, visitors and staff
- Understanding of what the NHS Constitution means to you, and your responsibilities to the public, patients and colleagues.
- Good understanding of health care relevant to safe and effective patient discharges
- Has the ability to recognise situations that require reporting and demonstrates the ability to escalate to a senior person where appropriate
- Work under minimal supervision to facilitate safe discharges

The Discharge Liaison Officer is a new and exciting role supporting and facilitating the safe and timely discharge of patients.
The key element of the Discharge Liaison Officer role will be to communicate with BTHFT colleagues, the MDT, patient, families, medical and nursing staff to ensure everyone is working to the same discharge plan. This list is not exhaustive and it is expected that there will be other key relationships which will develop within this role.
Main duties of the job
The post holder will need to be prepared to work 7.5hr shifts 5 days per week.
The role will be to support the MAIDT to facilitate effective discharge.
