Role summary
This part-time Clinical Lead role, Band 6 Community Nurse, is based at Tarporley Hospital and involves visiting patients in the Rural Response Hub area. The post holder will provide clinical leadership and oversight, focusing on preventing acute hospital admissions, supporting End of Life Care, and facilitating hospital discharges. The role requires strong communication and teamwork to deliver person-centred care at home.
Key responsibilities

Essential criteria
- First level registered general adult nurse
- Evidence of teaching and assessing
- SPC qualification in District Nursing or equivalent; demonstrating robust evidence of caseload management, community profiling and application of health needs assessment
- Proficiency in delivering high quality evidenced based care
- To be able to demonstrate continuing professional development
- Ability to work and communicate effectively within a MDT setting
- Significant experience of working within a community healthcare setting
- Experience of making autonomous decisions
- Demonstrate alignment with Trust Values (6 C's: Commitment, Communication, Care, Courage, Compassion, Competency)

Job summary
Due to investment and development in our new Community Response Hubs we are looking to recruit a Part time Community Nurse Band 6, Clinical Lead working 28.5 hours per week over three days.
Based at Tarporley Hospital visiting the patients of the Rural Response Hub area. Travel is an important part of this role. Access to transport for work purposes is essential.
Main duties of the job
The post holder will be a clinical resource, providing a visible clinical profile, having advisory, leadership and clinical oversight of the caseload.
The team focus is on prevention of unnecessary acute hospital admission, End of Life Care and Facilitating discharge from hospital.
By working collaboratively with the wider healthcare team we ensure person centred care for those patients whose preferred place of care is at home.
A high level of communication is essential for the role, with Team work at the core of our practice.
