Role summary
Join the Village and Repton Community Nursing Team as an enthusiastic and motivated Healthcare Assistant. You will provide holistic care to patients in their own homes and care home settings, performing duties such as wound care, catheter care, and administering injections. This role involves working within a community nursing team, assessing and evaluating individualised patient care, and supporting the District Nurse in managing the healthcare environment. Excellent communication skills and an NVQ Level 3 (or equivalent) are required.
Key responsibilities
- Assess, implement and evaluate individualised patient care in the most efficient and clinically effective way whilst maintaining the patient's privacy and dignity.
- Provide the delivery of high-quality care and actively support the District Nurse in the management of the health care environment.
- Work in close collaboration with the Integrated Community Teams.
- Carry out a plethora of patient care including wound care, catheter care and administration of injections.
Essential criteria
- Good standard of general education including English Language & Maths GCSE at level C or above, or equivalent
- NVQ Level 3 in Health and Social Care or equivalent
- Excellent verbal, written and interpersonal skills
- IT skills
- Ability to travel between care settings will be required
- Working in health and or social care environment
- Understanding of factors that influence health and wellbeing
- Understanding of the implications for, and responsibilities of the post for Risk Management and Safeguarding

- Abillity to problem solve
- Community experience
- MDT or integrated team working
- Palliative Care Experience
