Role summary
This Senior Community Nurse role in Tower Hamlets involves progressing clinical and leadership skills within the District Nursing team. You will be responsible for triaging referrals, conducting robust clinical assessments, and developing patient-focused care plans. The role focuses on enabling self-management and self-care for patients, working in partnership with locality teams to maintain effective patient flow. You will work shifts on a rota, covering Monday to Sunday.

- Experience and knowledge on holistic initial assessments.
- Experience of managing a team within the health care setting

We in the District Nursing team, have an ambitious vision for the Tower Hamlets community health service. We are striving to deliver high standards of care at the point of entry into our services, with the aim of improving access to the quality of life for all of our patients. One of our main aims is also to enable self care and independence.
Our staff are central to delivering the service vision and we are keen to recruit high calibre staff, who will be offered the support required to fulfill their roles as well as being provided with the training and experience to further their skills.
You will be a strong team player, an excellent communicator who is adaptable, committed, well organised and work well under pressure.
Having a sound clinical assessment skill and ability to effectively manage time is essential for the post.
Main duties of the job
- To work as part of a team ensuring all referrals are triaged and have a timely visit scheduled.
- To work as part of a team ensuring all new admissions to the Community Nursing services have a clinically robust assessment and patient focused care plan.
