Essential criteria

  • Degree or diploma in Occupational Therapy or Physiotherapy
  • HCPC registered, NMC Registered
  • Substantial post registration clinical experience in a similar role
  • Extensive experience of working within a multidisciplinary team
  • Evidence of appropriate postgraduate courses and CPD to support practice, to degree equivalent level
  • A broad range of clinical experience in a variety of community and inpatient settings
  • Knowledge and Abilities
  • Confident and competent leadership with the ability to work in diverse and complex situations
  • Ability to make clinical risk-management decisions in lone-working situations
  • Awareness of safeguarding policies and procedures
  • Skills and Aptitude
  • Excellent clinical skills
  • Highly developed verbal and written communication skills that will enable to post holder to work effectively across boundaries and in a variety of settings.

Job summary

The Home First East Team covers the Caradon area in East Cornwall and provides a responsive, multidisciplinary approach to supporting the intermediate care needs of our local population.

Our team comprises registered and non-registered nursing staff, therapists, and administrative colleagues who work together to deliver high-quality, person-centred care. We are committed to helping people remain as independent as possible within their own homes and communities, supporting admission avoidance, hospital discharge, and recovery from illness or injury.

We are looking for a motivated and compassionate practitioner to join our Community Response Team.

Main duties of the job

The successful candidate will be responsible for triaging referrals, undertaking holistic assessments, and developing and delivering evidence-based treatment plans for adults across the locality.

Working within patients' own homes, residential care homes and nursing homes, you will play a key role in supporting people to maintain their independence and avoid unnecessary hospital admissions.

You will work as part of a dynamic multidisciplinary team, collaborating closely with colleagues across community and acute services, Adult Social Care, primary care and voluntary sector organ
