Role summary
This role involves supporting patient discharge planning across sites, ensuring safe and timely patient releases from acute settings. Responsibilities include tracking referrals, recording patient data, and overseeing compliance with discharge initiatives. The coordinator will also update national reports and liaise with ward staff and external organizations to streamline the discharge process. Administrative tasks and continuous training are also part of the role.
Essential criteria
- GCSE English language level 4-9 (or equivalent)

- Previous health care experience experience
- Experience of using MS Office, Word, Excel, and outlook
- Experience of working with Multi-Disciplinary Teams and/or attending meetings
- Demonstrate an understanding of discharge planning
- Experience of working in the NHS, use of NHS systems and understanding of medical terminology
- Demonstrate understanding of GDPR and working with sensitive and confidential information
- Red2 Green/ Criteria to Reside / SAFER initiatives
- Local Discharge Planning processes
- A self-motivated, innovative, and a pro-active problem solver
- Able to meet tight deadlines, prioritise workload, and work under pressure
- Flexible in approach to meet the challenges of a trust wide post
- Demonstrate good standards of written and verbal communication
- Ability to communicate at all levels
- Ability to work independently or as part of a team

Job summary
- To provide a responsive patient discharge support service completing and tracking referrals from receipt to patient discharge.
- To support the wards on the safe and timely discharge of patients from the acute settings, including attending whiteboards.
- To record patient level data regarding Discharge Ready patients
- To oversee the completion and compliance with Red2Green and Criteria to Reside initiatives from a ward perspective.
- To update the sit rep report for reporting to national database, ensuring that it is accurate and in real time.
- Liaise with ward staff regarding changes in patient's clinical status affecting discharge and data recording.
- To support the Head of Site operations, Capacity and Discharge Planning, to perform all allocated administrative tasks required to meet the needs of the service.
- To undergo training as necessary to meet the needs of new developments within the service.
Main duties of the job
- Contribute to the department, ensuring that it operates to the highest standards, for discharge planning.
- Be responsible for ensuring that all duties are carried out in accordance with the correct procedures and policies.
- Ensure that E-Track information is correct and accurate.
