Role summary
This role is for a dedicated and forward-thinking senior Band 5 registered nurse to join a new self-care team. The successful candidate will work across the Fylde Coast, undertaking nursing duties in patients' homes or clinic settings, supporting patients towards self-management. Key responsibilities
- Undertake nursing duties in patients' homes or clinic settings as part of a community nursing service.
- Support patients towards becoming experts in their own health care interventions and self-management.
- Provide safe, effective, individualised care to patients and support their families.
- Support junior staff in delivering a high-quality service, including oversight of clinical caseloads, clinical supervision, and training.
- Facilitate learning and competence for staff and patients, ensuring they feel confident and able to optimise their skills.
- Actively promote a self-care model whilst ensuring core clinical skills are maintained.
Essential criteria
- Registered Nurse level 1
- Current NMC registration
- Evidence of ongoing professional development
- Supervisor/Assessor competence or willingness to undertake
