Role summary
This role involves delivering comprehensive, evidence-based diabetes services and providing leadership within the diabetes team. The post holder will manage a clinical caseload, assess health needs, and develop and evaluate care standards. Key responsibilities include providing expert clinical care, supporting staff development, and teaching patients and clinical staff. The role also involves collaborating across primary, community, and acute care settings to implement integrated care pathways.
Essential criteria
- Registered General Nurse
- Independent Nurse Prescriber
- Highly developed specialist knowledge achieved through First Level Degree or equivalent portfolio of evidence
- Recognised Diabetes Qualification
- Evidence of continual professional development and education
- Teaching Certificate or equivalent
- Willingness to undertake Masters level study/qualification
- Post registration experience
- Diabetes nursing experience
- Assessing and prescribing care plans for patients with complex needs
- Experience of provision of training/education to range of staff and patients
- Knowledge and Skills
- Ability to communicate complex & distressing information to clients & their carers in a sensitive e & supportive way at a level they can understand
- Ability to manage complex caseload of high risk patients
- Ability to take initiative, work autonomously, make decisions and prioritise workload
- Advanced training & utilisation skills in therapeutic/diagnosis procedures & equipment relevant to diabetes
- Knowledge & understanding of safeguarding practice, policy an guidance including statutory frameworks
- Excellent interpersonal and leadership skills
- Ability to use counselling skills

Job summary

Main duties of the job
To assess, plan and implement and evaluate evidence-based nursing care for people with complex needs for their Diabetes management. The post holder is required to provide expert clinical care for patients and to support other staff to develop their competencies in diabetes management. The post holder will be involved in teaching and training both formally to clinical staff and to patients.The post holder will be required to work collaboratively across primary and community care, and link to other specialists within the acute setting to implement the integrated care pathway for people with diabetes. The post holder will lead on audit, Service Development and Qualityimprovement work.
