Role summary

Join Kent and Medway NHS Social Care Partnership Trust as a Band 6 Community Mental Health Nurse within the Community Rehabilitation Team. You will deliver specialist, recovery-focused nursing care to adults with complex mental health needs, managing a community caseload to empower service users towards independence and improved quality of life. This role involves championing integrated physical and mental health care, guiding junior staff, and driving quality improvements in a supportive environment.

Essential criteria

  • Knowledge and application of risk assessment and risk management within community mental health settings
  • Ability to undertake and support physical health monitoring and interventions for service users with Serious Mental Illness (SMI).
  • Evidence of leadership, mentoring, supervision or supporting junior staff and students.
  • Knowledge of the Mental Capacity Act, NMC Code and relevant mental health legislation.
  • Excellent communication and partnership working skills with service users, carers and professionals.
  • Excellent communication skills, including a high standard of written and spoken English, with the ability to communicate complex and sensitive information effectively to service users, carers, colleagues and partner agencies.
  • Training, Qualifications and Registration
  • RMN or RNLD qualified
  • 2 years post-registration experience.
  • Practice Assessor.
  • Completed preceptorship.
  • Completed Edward Jenner Programme.

Desirable criteria

  • Evidence of CPD.
  • Minimum 2 years post registration experience, including experience at Band 5 level
  • Experience of assessing, planning, implementing and evaluating care for people with complex mental health needs
  • Experience of managing a caseload and working autonomously within a multidisciplinary team.
  • Experience of leading quality improvement projects, audits or service development initiatives
  • Experience supporting students, preceptees or newly qualified nurses
  • Community Mental Health or Community Rehabilitation experience
