Role summary
This Senior Support Worker role is within the Specialist Forensic and Community Wellbeing Team, focusing on the Severe Mental Illness (SMI) pathway in North Lincolnshire. The post involves delivering high-quality healthcare support under the direction of the Nursing Team and Allied Health Professionals in a primary care setting. Key duties include completing physical observations and health checks, promoting health and wellbeing, and supporting adults with mental health difficulties. The role requires working across pathways, contributing to integrated, person-centred interventions, and maintaining accurate patient records.
Essential criteria
- Demonstrable literacy and numeracy skills to Functional Skills Level 1 or equivalent
- NVQ Level 2 in a care-related subject or equivalent, or willingness to undertake a relevant Level 2 Apprenticeship
Desirable criteria
- GCSE Grade C or above or Functional Skills Level 2 or equivalent in English and Maths
- Demonstrate a keen interest in caring for others
- Experience in independently completing physical observations (blood pressure, pulse, BMI, etc.)
- Skilled in ECG application or willingness to undertake relevant training
- Previous experience of working as part of a team
- Previous experience of caring for patients in either a clinical setting or through personal experience
- Experience of mental health services
- Skilled in venepuncture or willingness to undertake relevant training
- Experience of working within primary care network mental health
- Understand the importance of maintaining confidentiality
- Good interpersonal skills
- Ability to communicate effectively both face to face and in writing
- Able to support and reassure patients and carers in distress
- Basic IT skills
