Role summary
This role involves delivering specialist learning disability and mental health support within a prison setting. Working as part of a multidisciplinary team, the post holder will collaborate with prison services and other stakeholders. The role requires providing expert advice and ensuring high-quality, coordinated care for vulnerable individuals.
Key responsibilities
- Manage a diverse caseload, undertaking assessments and delivering specialist nursing interventions.
- Support service users throughout their journey, ensuring person-centred care.
- Provide expert learning disability advice and contribute to multidisciplinary meetings.
- Ensure effective utilisation of clinical resources and report incidents compromising care.
- Coordinate services and aftercare arrangements for continuity of care and transition planning.
- Work within the Care Programme Approach framework to create seamless pathways.
Essential criteria
- Registered Learning Disabilities Nurse with the Nursing and Midwifery Council Register A Evidence of recent CPD and compliance with NMC revalidation Teaching Certificate including annual update (ENB 603) or related mentoring / coaching qualification
- Considerable experience of working in the learning disabilities field with a comprehensive knowledge in mental health experience of providing specialist interventions for those with learning disabilities and mental health problems, counselling techniques, CBT, motivational interviewing.
- Providing supervision and line management of staff where appropriate.
- Evidence of good nursing practice
