Role summary
This role is for an experienced frontline leader to join the Birmingham & Solihull Urgent Care Directorate as a Patient Flow Manager. You will be responsible for coordinating safe, timely, and effective discharge from hospital or inpatient care back to the community or another care setting. Key duties include planning discharges, coordinating professionals, managing complex cases, and ensuring safe aftercare arrangements.
Essential criteria
- Admission / Discharge experience
- RMHN/RLDN Degree level Qualification
- Knowledge of Services in the Patient Care Pathway
Desirable criteria
- System working - ICB, Ambulance, Police and other professionals
- Working within the Acute and Urgent care Environment
- Knowledge of Mental Health Act in Urgent/Acute Care

- Attending Ward Round
- Planning discharge early
- Liaising with other professionals including locality patient flow managers and Senior Urgent Care Nurse
- Attending deep dive meeting
- Attending Section 117 meeting
- Managing complex discharges
- Coordinating professionals
- Reducing delays in discharges
You will need:
Excellent communication skillsLeadership qualitiesAbilities to work collaboratively with partner agenciesExperience of managing change and leading staff.Ability to make sound clinical decisions in collaboration and offer support to staff as needed
