Role summary
This role is for a Band 6 Occupational Therapist to join the Integrated Front Door Team, focusing on improving urgent care pathways and supporting safe patient transitions. Key responsibilities
- Undertake holistic therapy assessments for patients presenting at the Integrated Front Door.
- Provide timely, evidence-based therapy intervention to support admission avoidance and early discharge planning.
- Work collaboratively with the multidisciplinary team to support clinical decision-making and patient flow.
- Identify and facilitate referrals to appropriate onward services, including community pathways and social care.
- Manage a delegated caseload in a fast-paced front door environment and contribute to board rounds and MDT discussions.
- Maintain accurate and timely clinical records and support service improvement.
Essential criteria
- Diploma/degree in Occupational Therapy
- Current registration with HCPC
- Evidence of specialist post graduate training and education with older adults.
- Experience of working in multi-disciplinary / multi-agency teams
- Significant Post graduate occupational therapy experience in a variety of clinical settings with experience in a community setting.
- Must be able to meet the travel requirements of the post
- Understanding of clinical governance and risk management and their implications for the service.
- Ongoing awareness of recent developments in practice and research including NSF's/NICE guidelines.
- Excellent time management skills.
- Effective communication within the team.
