Role summary

Key responsibilities

  • Undertake holistic therapy assessments for patients presenting at the Integrated Front Door.
  • Provide timely, evidence-based therapy intervention to support admission avoidance and early discharge planning.
  • Work collaboratively with the multidisciplinary team to support clinical decision-making and patient flow.
  • Identify and facilitate referrals to appropriate onward services, including community pathways and social care.
  • Manage a delegated caseload in a fast-paced front door environment and contribute to board rounds and MDT discussions.
  • Maintain accurate and timely clinical records and support service improvement.

Essential criteria

  • Diploma/degree in Occupational Therapy
  • Current registration with HCPC
  • Evidence of specialist post graduate training and education with older adults.
  • Experience of working in multi-disciplinary / multi-agency teams
  • Significant Post graduate occupational therapy experience in a variety of clinical settings with experience in a community setting.
  • Must be able to meet the travel requirements of the post
  • Understanding of clinical governance and risk management and their implications for the service.
  • Ongoing awareness of recent developments in practice and research including NSF's/NICE guidelines.
  • Excellent time management skills.
  • Effective communication within the team.
