Role summary
This role is for an enthusiastic and organised Health and Social Care Co-ordinator to join the locality team in Weymouth and Portland. The successful applicant will coordinate communications and meetings between various health and social care services to provide seamless care for frail and vulnerable individuals. It involves working with clinicians, patients, carers, and agencies to support effective assessment, care planning, and prevent unplanned admissions. Excellent organisational and communication skills are essential for this pivotal role in developing integrated services.
Essential criteria
- NVQ level 3 or equivalent level of knowledge in office procedures
- Evidence of experience of a range of administrative systems and software programmes
- Job specific experience
- Previous experience in an NHS / Primary Care / Local Authority role
- Experience of working under own direction
- Experience in a customer focussed environment
Desirable criteria
- Experience of supporting service improvement
- Managerial experience
- Experience of planning and organising complex meetings/ agendas and taking minutes
- Information technology
- Evidence of working with IT systems including Word, Access, Excel
- Evidence of ability to support collation and analysis of data
- Business travel
- Subject to the provisions of the Equality Act, able to travel using own vehicle on Trust business
- Personal Qualities/attributes
- Excellent written and verbal communication skills
