Role summary
This is an exciting opportunity for an experienced Nursing Associate to work as part of a friendly and dynamic multi-disciplinary care team within residential home settings. The role involves direct patient contact, providing high-quality and compassionate care, and working at a level above Healthcare Support Workers. Care Home Practitioners also provide training and support to care homes and collaborate closely with GP surgeries and other health and social services.
Key responsibilities
- Carrying continuing responsibility for ensuring adequate training and guidance is carried out within the Residential care setting.
- Undertaking tasks delegated by a Registered Practitioner.
- Assisting in patient assessment and making referrals to the Registered Practitioner and other MDT's.
- Performing higher clinical skills such as catheterisation, wound care, phlebotomy, and continence assessments within residential homes.
- Leading on advance care planning/admission avoidance and providing supervision and teaching to Care Home staff.
- Working as an autonomous practitioner within the Community Nursing Team, supporting care delivery and collaborating with system partners.
Essential criteria
- Associate Practitioner Qualification
- NVQ 3 or above
- Phlebotomy
- Wound care management
- Catheter care
- Diabetes management

- Community Nurse experience
- Care home knowledge
- Advanced care planning experience
- Compression bandaging skills
