Role summary
This role involves providing community care and support to patients in their own homes, aiming to prevent hospital admissions and facilitate early discharge. As a Band 3 Community Support Worker, you will assist Band 5 Community Staff Nurses by visiting patients to monitor observations, obtain blood samples, provide wound care, and offer education and support. The job requires lone working within the locality and access to a vehicle for travel.

- NVQ/Diploma Level 3 in Care
- CPD Diary effectively demonstrating levels of learning and reflection from courses / experiences
- Skills, Knowledge & Abilities
- Knowledge of Health Promotion
- Good communication skills.
- Awareness of Health and Safety and Manual Handling requirements
- Experience of working in a Health/Social Care/ related environment
- Education & Qualifications

Job summary
We are a community service providing care and support for patients in their own homes preventing hospital admission and facilitating early hospital discharge.
We work closely with primary and secondary care taking referrals from both hospitals and GPs.
We have a good rapport with our patients and get excellent feedback so job satisfaction is high. We take pride in enabling patients to stay at home when acutely unwell.
Our main priority for this job role is to find people with the right values, behaviours and attitudes that match our own. Previous experience is an advantage but we can provide training if necessary.
