Role summary

This is an exciting opportunity for a Band 6 Clinician to join the County South Crisis Team as a Crisis Care Practitioner. The team provides rapid response and intensive home support for adults experiencing acute psychiatric crises, working collaboratively with community teams and partner organisations. The role involves providing mental health assessments, triaging referrals, and delivering effective clinical services within the Crisis Resolution/Home Treatment Service. The successful candidate will be an experienced Mental Health Practitioner with proven post-qualification experience and sound knowledge of risk assessment.

Key responsibilities

  • Devolved Management, Leadership and development role
  • Ensuring and supporting the effective delivery of clinical practice by the CRHT team
  • Provide mental health assessments within a variety of settings
  • Triage referrals coming into the service
  • Working collaboratively and in partnership with other statutory, non-statutory and voluntary teams, agencies and organisations
  • Ability to demonstrate clinical expertise in mental illness / health
  • To use interpersonal skills in diffusing potentially emotive situations while undertaking assessments and pathway planning within acute settings
  • To demonstrate high level communication skills.
  • Working to a rota shift system covering 24 hours, 7 days a week
  • Travel is expected as an essential part of the role

Essential criteria

  • Professional qualification in Mental Health Nursing or Occupational Therapy, at Diploma or degree level
  • Evidence of learning and development post qualification
  • Relevant post registration experience in adult mental health.
  • Experience of multi-disciplinary working
  • Experience of working in acute mental health care
  • Competency in completing comprehensive mental health assessments and assessments of risk.
  • Experience in the development of robust care plans and risk management plans
  • Understanding of responsibilities in relation to the mental health act, mental capacity act, CPA, and safeguarding
  • Competency in meeting the expected performance and data quality targets for the role in a timely manner and an understanding of the importance of this.

An exciting opportunity has arisen for a Band 6 Clinician to join the County South Crisis Team as a Crisis Care Practitioner.

The CRHT team provide rapid response and intensive home support for adults who are experiencing an acute psychiatric crisis. We also work with our community teams and partner organisations to provide a range of support options for people experiencing mental health crisis.

Main duties of the job

We are seeking an experienced Mental Health Practitioner to join our team of Consultant Psychiatrists, Junior Doctors, Non-Medical Prescribers, Nurses, Social Workers and Support Workers providing rapid response for adults who are experiencing an acute psychiatric crisis.

You will be an experienced Clinician and will have proven post qualification experience. You will have a sound knowledge of risk assessment and demonstrate sound evidenced based practice. Experience of working in a crisis team or community setting will be advantageous.
