Role summary

This is an exciting opportunity for a Band 5 NMC Registered Nurse to join the Out of Hours Community Nursing team in South and Central Hubs. The role involves providing exceptional care to patients in their homes or residential settings, and potentially in clinics. You will assess care needs, develop and implement care plans, and undertake various clinical duties. The position offers training and development opportunities within a supportive team.

Key responsibilities

  • Deliver care to patients in their own homes, residential homes, or clinic settings.
  • Assess care/support needs and develop, implement, and evaluate care/support packages.
  • Prioritise treatment and care for individuals based on their health status and needs.
  • Provide palliative and end of life symptom management, catheter management, and wound management.
  • Undertake Tissue Viability Risk Assessment and have supervisory responsibilities as required.

Essential criteria

  • Evidence of/willing to undertake post-registration training relevant to the post
  • Experience in a community setting or in palliative care, catheter care or similar
  • Degree in Adult Nursing or equivalent
  • Current NMC registered general nurse (degree in nursing)
  • UK Car and driving licence
  • Relevant experience
  • Evidence of clinical knowledge relevant to the post, with knowledge of care planning. Understanding of Information Governance, Data Protection, NMC standards and Safeguarding Vulnerable Adults
  • Flexibility to work across 7 days
  • Basic IT skills

Job summary

We are offering an exciting opportunity for a Band 5 NMC Registered Nurse to join our Out of Hours Community Nursing team in the South and Central Hubs which are based in Norton Canes and Stafford.

You will be committed to providing exceptional care in patients own homes demonstrating a wide-range of clinical skills. Community services are admitting patients with a more complex acuity than ever before with increasingly specialist treatment requiring care from highly skilled staff.

You will also have an opportunity to undertake training and development to ensure you are well equipped for the role, with opportunity to advance further. Extensive in house support is available from the teams own Lead Practitioner and Community Educator.

The shifts are 18.00-06.15 and 20.00-08.15. The shifts cover 7 days per week evenings and nights to fully embrace the ethos and values of patient centred care. The team are friendly and supportive, with a key focus on staff wellbeing and safety.

Due to the nature of the role you will be expected to travel independently over a large geographical area and will ideally require a valid driving licence and access to a car.
