Role summary
This role involves working as part of the Essex Wellbeing Service Tobacco Control Team to provide evidence-based smoking cessation support to residents. You will manage caseloads, engage with individuals via telephone, and assess their readiness for change to improve their lifestyle. The position focuses on supporting vulnerable adults and various communities through the Single Point of Access and clinic settings. It also includes assisting in community health events and accurately recording client information.
Key responsibilities
- Manage caseloads efficiently and in line with service priorities.
- Engage with individuals by telephone, using a holistic triage approach to assess readiness for change and identify support needed.
- Support the SPA in onboarding and training new smoking specialist services.
- Deliver training to SPA Care Navigators on new smoking specialist services.
- Support all individuals, with a strong focus on vulnerable adults, MH, seldom heard communities, and LTCs.
- Ensure all interactions comply with relevant policies, procedures, and professional standards.
Essential criteria
- Relevant Foundation degree in health and social care or community development Or Relevant Diploma plus relevant experience health improvement and community development
- Making Every Contact Count training or willingness to undertake it
- Evidence of working in public health.
- Clear understanding of behaviour changes methods and evidence of applying it
- Clear and demonstrable understanding of health inequalities and the main issues of health affecting the local community.
- Evidence of experience in partnership working and community development
- Experience of multi-agency working
- Ability to support behaviour change by applying behaviour change techniques.
- Able to manage own time.
- Excellent communications skills both written and verbal.
- Ability to interpret individual client data for optimal intervention planning
- Proactive in working collaboratively to achieve individual outcomes.
- Ability to be developmental and creative in approach.
- Solution-focused attitude.
- Must be IT literate with experience of all Microsoft Office applications.
- The Post holder be required to work from multiple sites within the locality as well as working from home
- Working in a mobile and agile way
- Flexible working within the operating hours of the service which will include evenings and weekends.
- Car owner/driver or ability to travel independently.
Desirable criteria
- Mental Health First Aid Level 2 Stop Smoking
- Willingness to undertake further training as appropriate
