Role summary
The Community Governance and Quality Facilitator will promote and establish clinical governance principles within Community Services. This role involves coordinating, monitoring, and evaluating governance processes to ensure safe, effective, and high-quality patient care. The post holder will provide specialist advice on governance, risk management, and patient safety, identifying risks, investigating incidents, and monitoring compliance. They will also facilitate continuous improvement and contribute to a positive learning and safety culture.
Essential criteria
- Educated to degree level or equivalent experience in a healthcare, governance, or business management field.
- Willingness to undertake further professional development
Desirable criteria
- Qualification or specialized training in clinical governance or quality improvement.
- Qualification or specialised training in Patient Safety Incident Reporting Framework (PSIRF)
- In-depth understanding, NHS incident reporting (e.g., Datix) and incident responses and SEIPS methodologies.
- Ability to monitor action plans and compliance to clinical governance and quality frameworks.
- To apply knowledge and skills to prioritise and meet current and emerging work demands
- Knowledge and understanding of a clinical governance function within the NHS
- Able to prepare and deliver reports, training packages and presentations to community nursing services.
- Familiarity, or knowledge about NHS regulatory bodies, compliance, and clinical effectiveness requirements (e.g., NICE guidelines).
- Knowledge of clinical audit and effectiveness systems (AMat) and methodologies.
- Significant experience working within clinical governance, quality, patient safety or healthcare management functions
- Practical experience in procedural document management and handling sensitive information.Essential
- Experience in influencing teams and facilitating culture change
- Experience of proven ability to produce high-quality written reports in various formats
- Experience analysing incidents and identifying themes/trends
- Experience that demonstrates a commitment to patient-centred care and improving patients' outcomes
- Experience or awareness of national and Trust initiatives that impact upon the local clinical governance and quality improvement agenda
- Experience applying PSIRF methodologies
- Experience in using departmental systems and the internet/intranet for tasks completion
- Experience or strong understanding of quality improvement initiatives, such as handling patient complaints, incident response, and managing/handling to patient's feedback

The post holder will facilitate the Clinical Governance/quality Management, to promote and establish the key principals of clinical governance within Community. The post holder will facilitatethe governance objectives and targets to ensure the delivery of a quality service. The post holder will work in collaboration with the Community risk and governance Lead and community leads.
Main duties of the job
The Community Governance Facilitator will provide specialist governance, patient safety, risk management and quality improvement expertise to Community Services. The post holder will coordinate, monitor and evaluate governance processes, supporting services to deliver safe, effective and high-quality patient care.
The post holder will provide specialist advice and guidance on governance, risk management and patient safety matters, providing governance expertise to managers and clinical teams to identify risks, investigate incidents, monitor compliance and implement learning. They will analyse complex information from multiple sources, identify trends and provide assurance regarding quality, safety and regulatory requirements.
