Role summary
This role involves coordinating and delivering engaging, person-centred activities for patients living with dementia in a specialist unit. The coordinator will plan varied individual and group activities to promote independence, social interaction, cognitive stimulation, and emotional wellbeing. Working within a multidisciplinary team, the post holder will support personalised care plans and contribute to a positive and therapeutic environment. The aim is to enhance the wellbeing, dignity, and hospital experience of patients with dementia.
Key responsibilities
- Planning and delivering a varied programme of individual and group activities tailored to patients' interests, abilities, and care needs.
- Promoting social interaction, cognitive stimulation, emotional wellbeing, and independence to improve quality of life and reduce isolation.
- Working closely with nursing staff, therapists, the Complex Dementia and Delirium Team, patients, families, and carers to support personalised care plans.
- Encouraging and motivating patients to participate in recreational, therapeutic, and leisure activities within a safe and supportive environment.
- Monitoring patient wellbeing, recognising changes in condition, and escalating concerns to the clinical team as appropriate.
- Maintaining accurate records and providing feedback on patient engagement, participation, and wellbeing.
Essential criteria
- NVQ level 3 OR relevant experience
- Experience of working with service users with dementia
- Ability to work as part of a team
- Ability to communicate effectively with patients/relatives/carers
- Positive and enthuasiatic approach to work
- Excellent written and verbal communication skills
- Ability to work flexible shift patterns and times to suit the needs of the patient group
Desirable criteria
- Experience of using activities in a care/public service environment
