Role summary
This role is for an enthusiastic Health Care Assistant to provide safe and effective care for patients in their homes as part of a Community Nursing Service. The successful candidate will work independently, providing personal and palliative care, managing clinical supplies, and adhering to Trust policies. They will also monitor skin integrity, provide health screening, and assist with clinical interventions as directed by Registered Nurses.
Key responsibilities
- Assist the Community Nursing Team in providing nursing care and support to patients and families in the community setting.
- Provide personal care and support to palliative patients in their own homes.
- Work alone and unsupervised, demonstrating autonomy in the community setting.
- Be responsible for ordering clinical supplies and stationery for the Community Nursing Team.
- Monitor skin integrity of patients at risk of pressure ulcers and provide basic preventative information.
- Provide clinical intervention for care of patients as directed by Registered Nurses, following training and assessment of competencies.

- GCE/GCSEs Grade A*- C/level 9- 4 in Maths and English or Equivalent or Willingness to undertake Level 2 Functional Skills Qualifications in Maths and English

- Demonstrable experience of working in a healthcare setting
