Role summary
This role involves providing care and support to individuals in the community, focusing on maximizing their independence after acute illness or hospitalisation. You will undertake short-term care as defined in service plans, contribute to assessments, and monitor progress. The position requires working as part of a team, including lone working, across Torbay and South Devon to deliver integrated health and social care. This is a part-time permanent contract with flexible hours, including evenings, weekends, and bank holidays.
Essential criteria
- Experience of personal care and/or clinical procedures, to NVQ2 level qualification OR equivalent knowledge
- Basic IT Skills - Microsoft Word, Outlook, Excel
Desirable criteria
- NVQ3 or QCF or Diploma Level 3 in Health & Social Care
- Basic Food Hygiene Certificate
- Knowledge and experience
- Able to demonstrate a good level of literacy, numeracy, verbal and written skills, including record keeping skills
- Demonstrate skills in written and spoken English, adequate to enable the post holder to carry out the role effectively
- Excellent Interpersonal and communication skills
- Able to communicate with individuals of varying levels of understanding
- Ability to assist service users in the activities of daily living whilst promoting independence
- Knowledge/understanding of equipment used to promote independence.
- Ability to carry out safe practice when moving and handling users and equipment.
- Understanding of relevant health and safety legislation for moving and handling

- Experience of working in a caring environment
- Able to demonstrate an understanding of the rights of the Service User
- Able to demonstrate a commitment to working with older adults
- Able to demonstrate an understanding of health and safety and safeguarding issues
- Experience of working in community setting and lone working
- Previous experience of working in a multidisciplinary team
- Experience of updating service users' plans
- Experience of working within and applying safeguarding adults' procedures
- Experience of working with a rehabilitation ethos
- Experience of working with people at end of life
- Requirements due to work environment/conditions
- Ability to work flexibly including unsocial hours and shift patterns including weekends and Bank Holidays
- Have full UK driving licence and access to a vehicle

Job summary
Our short-term services are recruiting for a Support Worker to join our team on a part-time (36 hrs) permanent contract were you will provide care and support to people in the community with a focus on maximising independence, and delivering a high standard of care to a variety of people, this can also include end of life care.
Following a period of acute ill health or hospitalisation, you will undertake short term care as defined in care & short term service plans or delegated task from Therapist/Nurses, and contribute to the assessment process and ongoing monitoring and review of peoples goals and reporting any changes to a persons medical condition.
This is an exciting development of services to help prevent avoidable hospital admissions working alongside intermediate care teams and other community teams.
Main duties of the job

You will be working as part of a team, this will include lone working within the community.
