Role summary
This role is for a Mental Health Nurse with a passion for improving the quality of life for people living with dementia. You will join the High Peak and North Dales Inreach and Home Treatment team in Bakewell, providing person-centred, non-pharmacological support to older adults experiencing mental health crises. The service aims to prevent hospital admissions and facilitate early discharges, operating 365 days a year. As a Lead Nurse, you will manage complex cases within a multi-disciplinary team and make critical clinical decisions.
Essential criteria
- Recent and relevant, post-qualification nursing experience as RMN
Desirable criteria
- Community experience
- Experience working with older adults
- Experience in working with people with Dementia
- Mentorship/SSSA trained or willing to complete training
- Supervision trained
- Knowledge of MHA/Capacity act/Legal frameworks
- Able to meet the travel requirements of the post

Then the High Peak and Norh Dales Inreach and Home Treatment team welcomes your application!
Based in Bakewell, our service promotes a person-centred and non-pharmacological approach to support older adults living with a diagnosis of dementia, who may be experiencing or approaching a crisis in their mental health.
We cover the High Peak and North Dales area of Derbyshire.
The aim of the service is to assist in maintaining the independence and dignity of those living with dementia, with the ultimate goal of preventing hospital admission where possible.
In addition the IRHTT can support early discharge from hospital, working closely with carers and relatives to facilitate this.
The service operates a service 365 days a year, Monday-Sunday, 8am-8pm and 9am-5pm at the weekends and Bank Holidays.
Main duties of the job
You will:
- Employ a wide range of skills, building on and developing their expertise in managing complex cases as part of a wider multi-disciplinary team, which includes Occupational Therapy, Physiotherapy, Nurses, Dementia Specialist Support Workers, Psychology and a Consultant Psychiatrist.
