Role summary

This role is for a full-time Band 7 Specialist Therapist in Frailty, working within a multi-disciplinary hospital/community team at Rochdale Infirmary. The post holder will support frail patients in both hospital and community settings, promoting a home-first approach. Key responsibilities include delivering the frailty service, managing patient health needs with carers, and collaborating with multidisciplinary teams to shape high-quality, patient-centered services.

Essential criteria

  • Registration with the appropriate professional body ie NMC/HCPC
  • Degree qualification or equivalent
  • Recognised teaching/mentor qualification

Desirable criteria

  • Previous experience of team leadership
  • APLS / ALS
  • Level 7 clinical skills
  • V300 (non medical prescribing) or willingness to undertake
  • Knowledge, Training & Experience
  • Substantive post registration experience
  • Extensive knowledge/experience in urgent and or community setting
  • Knowledge and understanding of local health and social care pathways
  • Understanding of long term conditions including frailty and end of life processes
  • Evidence of planning and leading service development
  • Previous experience or involvement in the development of service policies and protocols
  • Skills & Abilities
  • IT skills
  • Promotes and support own and others CPD
  • Ability to travel to different geographical locations across the trust footprint in line with service needs.
  • Full UK driving license with willingness to use own car for work purposes
