Role summary
This is an exciting opportunity for a compassionate and motivated Band 5 Community Nurse to join the Community Adult Twilight & Night Service in Brent. The role involves providing high-quality, responsive nursing care to patients in their own homes, aiming to maintain independence and prevent hospital admissions. The nurse will work autonomously within a supportive multidisciplinary team, delivering general and specific care under supervision. Shifts include twilight (4:00pm - 12:00 midnight) and night (12:00 midnight - 8:30am) across Brent's five localities.

- Clinical nursing experience
- Working with people with complex health needs
- Experience of undertaking assessment by utilising evidence-based care.
- Previous experience in the Nursing process, care planning and fluent in record keeping
- Experience of providing holistic care that involves the multidisciplinary team
- Experience of training other students and HCAs
- Participation in audit; research projects, service development
- Post registration experience in a NHS community environment
- Skills and Knowledge
- Knowledge of out of hospital care and the role of the Community Nurse
- Knowledge of reasonable response to service user needs prioritising and dealing with those needs with a required urgency.
- Knowledge of risk involved in working in the community
- Knowledge of evidence based practice
- Knowledge of management of long-term condition e.g Diabetes and an understanding of the concept of self-management
- Knowledge of infection control
- Literacy and numeracy skills
- Ability to listen effectively and ability to give constructive feedback to clients and colleagues
- Knowledge of recent government legislation and changes in community care delivery
