Role summary
This role involves managing a specialist physiotherapy caseload within the Integrated Community Stroke and Neuro Service (ICSNS) across East and North Hertfordshire. The post holder will provide specialist assessment and treatment for patients with various neurological conditions in community settings and patients' homes. It requires working as part of a multidisciplinary team, supervising junior staff, and participating in a weekend rota. The service focuses on delivering high-quality, person-centred neurological rehabilitation.
Key responsibilities
- Manage a specialist caseload of patients with neurological conditions, providing assessment, treatment, and rehabilitation.
- Deliver person-centred care within patients' homes, outpatient clinics, and a Neurological Rehabilitation Unit.
- Work collaboratively with health and social care colleagues, carers, and families to support patient goals.
- Participate in multidisciplinary team meetings and contribute to coordinated care planning.
- Provide supervision and support to junior staff and students.
- Contribute to service development activities, including audit and quality improvement.
Essential criteria
- Physiotherapy degree or equivalent
- HPC registration
- Experience of working with neurological patients in a community / inpatient setting
- Documented evidence of professional development and reflective practice in relevant aspects of neurological rehabilitation: e.g. attendance at in-service training sessions, reflective practice etc
- Ability and experience of carrying out a comprehensive neurological Physiotherapy assessment in order to formulate a comprehensive neurological treatment plan and goals.
- Knowledge and experience of using a wide range of therapeutic Physiotherapy interventions for the treatment of Neuroloogical patients
- Knowledge and experience of using a range of recognised outcome measures and goal setting skills as appropriate to the rehabilitation of Neurological patients
- Good understanding of current NHS issues, professional standards, CPD, clinical governance
- Physical skills, dexterity, coordination and sensory skills for assessment and manual treatment of patients. A good ability with specific handling and sensory techniques specific to the treatment of patients with neurological dysfunction.
- An ability to demonstrate clinical problem solving in complex cases.
- Ability to prioritise patient workload
- Ability to carry out patient manual handling tasks.
Desirable criteria
- Attendance at relevant post-graduate neurological training courses e.g. Normal movement, spasticity management, splinting, cognition and perceptual assessment, fatigue management
- Experience of supervising and appraising other staff e.g. rehabilitation assistants
- Experience of supervising students.
- Experience in delivering training to Physiotherapists and other members of the MDT
