Role summary

This is an exciting opportunity for an experienced Senior Therapist to join the Urgent Community Response (UCR) Team in Oldham. The role involves working as part of a multi-disciplinary team to drive changes in community care, supporting the hospital avoidance agenda and the NHS Long Term Plan. The successful candidate will provide holistic therapy intervention to reduce unnecessary hospital admissions and enable patients to remain in their own homes. They will work independently while being an integral team member, demonstrating strategic planning and a strong clinical background in urgent care.

Essential criteria

  • Occupational Therapy or Physiotherapy Degree
  • professional registration
  • HCPC registration

Desirable criteria

  • RCOT or CSP registered
  • Previous experience within urgent / emergency care
  • Experience of management and / or leadership
  • Conduct comprehensive therapeutic assessment including assessing for mobility aids / stairs / equipment / dressing aids
  • Ability to effectively discharge plan complex patients
  • Able to manage own time effectively including planning and prioritisation of both clinical caseload and leadership responsibilities
  • Experience of participating in clinical audit
  • Engaged in clinical supervision

Job summary

We have an exciting opportunity for a self-motivated, experiencedTherapist to join our UCR Team in Oldham Community.We need people who share our excitement, embrace different ways of workingand are able to work flexibly across organisational boundaries supporting the NHSLong Term Plan and admission avoidance agenda.Position The post holder will work as part of the multi-disciplinary team, driving the changes to community care response to support the hospital avoidance agenda. They will work in line with the agreed strategic objectives of Northern Care Alliance.The aim of the team is to reduce unnecessary hospital admissions with an aim of avoiding attendance to the Emergency Department, by providing holistic therapy intervention and a person centred approach support in order to enable patients to remain in their own home or usual residence. The team works closely with community services to support the home first model.

Main duties of the job

The successful candidate will be expected to work independently, but also as an integral member of the multidisciplinary team within the UCR team. The candidate will be able to demonstrate strategic planning and have a strong clinical background in Urgent Care or a similar field. Excellent time management
