Role summary
This role is for a passionate and qualified Psychiatric Nurse to join the Oak Team, part of the City and North East Oxfordshire AMHT. You will act as a key worker for patients with severe and enduring mental illness, conducting assessments, collaborative care planning, and reviews. The position involves undertaking bio-psycho-social assessments, offering brief evidence-based interventions, and completing duty tasks including triaging new referrals. You will work within a dynamic multi-disciplinary team, focusing on person-centred care and risk management.

- Occupational Experience
- To have experience of working within a multidisciplinary team
- To have highly extensive experience which should include community and working with mental health
- To have a good knowledge and interest in community care and have specific clinical expertise in mental health
- Understand and have experience of Risk Management
- Experience of supervising other staff
- Understand and have experience of Risk Management
- Good knowledge of community services
- Awareness of National and local issues concerning mental health
- Experience of supervising student placements.
- Experience of audit
- Skills required - IT / Admin
- To be IT literate
- To have excellent English written communication skills

Our service includes three locality-based teams aligned with GP practices. You will join a dynamic multi-disciplinary team, working alongside a range of clinical professionals and closely with third sector partners to provide high-quality, holistic, person-centred care.

This is a full-time role (37.5 hours, Monday-Friday, 09:00-17:00).
Due to the nature of the role, the ability to travel independently across Trust sites and to conduct home visits is essential and not all areas are accessible via public transport.
