Role summary
This role is for a Specialist Community Public Health Nurse (School Nurse) within the 0-19 Healthy Child Programme in Cambridgeshire North Locality. The successful candidate will promote good health and prevent ill health in young people, families, and communities by identifying health needs and implementing care plans. The position involves working within a multidisciplinary team, assessing needs, and collaborating with various agencies to improve health outcomes for children and families. Key responsibilities
- Work as part of a multidisciplinary team to identify health needs of children and young people and develop strategies to address them.
- Assess, plan, implement, and evaluate individual health care plans, managing complex health needs in schools.
- Provide and promote appointment clinics, parent/carer support groups, home visits, and health needs assessments.
- Act as a health link for other agencies and work closely with partners to promote the wellbeing of children, young people, and families.
- Participate in all statutory processes in respect of safeguarding, including child protection case conferences and court attendance.
- Identify children with special educational needs and disability, facilitating support and referrals to maximise developmental potential.
Essential criteria
- Specialist Practitioner degree/diploma post registration SCPHN
- Registered Nurse with UK NMC (RGN/RMN/RSCN/RN-LD)
- Mentor/998 or equivalent teaching qualification
- Experience with children and their families
- Experience of working in the community
- Experience of working in a multi-disciplinary setting
- Experience of Safeguarding children and families
- High level of clinical skills, with the ability to identify, respond to and evaluate health needs
- Commitment to the development of excellent services, which focus on improved outcomes for children
- Current knowledge of local and national policies informing public health practice
- Evidence of partnership working in the community
- Good standard of keyboard skills and understanding of clinical systems
Desirable criteria
- Recognised leadership course
- Evidence of completion of migratory portfolio to demonstrate dual qualification or willing to work towards
- Experience of mentoring/supporting colleagues
- Leadership and management of staff
- Evidence of contribution to service redesign/development
- Audit and data analysis
