Role summary
This is an exciting opportunity for a highly motivated Community Staff Nurse to join the Community Nursing Team in North Shrewsbury. The role involves providing planned care to patients in their own homes, supporting timely discharge, and preventing hospital admissions. The successful candidate will be caring, motivated, and committed to interdisciplinary team working and professional development. A valid driving licence and access to a vehicle are essential for this community-based position.
Essential criteria
- RGN
- NMC Registered
- Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes
- Post registration general experience, previous community experience
- Experience of working in a multi disciplinary / multi agency health/social care environment.
- Experience and knowledge of basic administration systems including Microsoft office
- Experience of successful partnership working arrangements with other services
- Knowledge and Skills
- IT skills or willingness to undertake RIO Training
- Up to date with Mandatory Training
- Uses evidence based practice
- Works within own defined competencies
- Awareness of changing healthcare factors influencing change Demonstrates tact and diplomacy
- Well refined communication (verbal and written), interpersonal skills - ability to gain rapport at all levels
- Engaging and empowering approach that seeks to work with others and deliver outcomes in a successful and well informed manner

The team provides nursing care to patients within their own homes or chosen place of residence offering an alternative to hospital admission, supporting timely discharge and enabling people to realise their potential.
We are committed to clinical support, CPD & training. The post forms part of a 7 days, 8am to 6pm service.
