Role summary
This role is for an enthusiastic and motivated registered nurse to join the community palliative care team in Thundersley/Benfleet as a Band 6. The post holder will manage a caseload of end-of-life patients, working as a key member of the Palliative Care Team to deliver care alongside nurses and therapists. The aim is to reduce inappropriate secondary care admissions through advanced care planning and high-quality end-of-life care. The nurse will provide holistic advice and support, working autonomously to manage symptoms and liaise with other healthcare professionals.
Key responsibilities
- Hold a caseload of end of Life patients.
- Work as a key member of the Palliative Care Team delivering care alongside nurses and therapists to end of life patients.
- Work to reduce all inappropriate admissions to secondary care through the use of advanced care planning and practical high quality end of life care.
- Provide holistic advice and support to palliative patients in the community.
- Work autonomously providing palliative care in the community.
- Manage symptoms as a key feature of the position.
- Liaise with other health care professionals and agencies to provide seamless care.
Essential criteria
- Enthusiasm for the discipline
- Experience with palliative care patients
- Minimum 18 months post registration experience
- Ecxcellent communication skills

- Experience of community/lone working
- Use of SystmOne
- Palliative care qualification/course
