Role summary
This Administrator role supports the Community Transfer of Care Hub (CToCH) in North Somerset, facilitating safe hospital discharges and ensuring patients receive appropriate community care. The post holder will provide comprehensive administrative support to clinical and administrative teams, manage patient records, handle enquiries, and contribute to data collection. This collaborative role involves working with various healthcare and social care partners to improve patient experience during their discharge journey.
Key responsibilities
- Provide comprehensive administrative support to health and social care professionals, hospitals, GPs, service-users, their relatives and carers.
- Answer telephone enquiries in a friendly and professional manner, ensuring effective communication.
- Maintain accurate and up to date patient records and data in Sirona IT systems, electronic patient records, databases, and spreadsheets.
- Support the delivery of the admin component within the Community Transfer of Care Hub (CToCH).
- Work alongside the Admin lead to support Discharge to Assess pathways from Acute settings.
Essential criteria
- Functional literacy and numeracy level 3/ GSCE grade A-C or equivalent including English Language and Mathematics
- NVQ 3 Business and Administration or equivalent experience
- Ability to take responsibility for tasks and to work autonomously if needed
- Ability to manage time, work under pressure and build efficient working practices
- Experience of working in a demanding office environment and meeting deadlines
- Trained in a range of systems /IT Clinical System and Microsoft Office
- Able to handle sensitive and confidential issues and recognise the need for discretion and confidentiality
- Call handling experience
Desirable criteria
- Previous acute or community care experience
- Proven ability to maintain effective workload prioritisation
- Previous experience of Cloud based telephony systems
- Previous use of EMIS

Our Community Transfer of Care Hub (CToCH) is looking for an organised Administrator to support our clinical and administrative teams in delivering Discharge to Assess pathways, which helps people leave hospital safely and access the right care in the community.
You'll play a key role in supporting timely hospital discharges, improving patient experience, and collecting data that helps demonstrate the impact of our service.
Working with colleagues across hospitals, social care, primary care, community services and the Integrated Care Board (ICB), you'll support both patients and their families throughout their discharge journey. This is a collaborative multi-disciplinary team, where you will help todevelop new ways of working that ensure people leave hospital safely, promptly and with the right support in place.
