Role summary
This role is for a Registered Nurse to join the Amber Valley Community nursing service, providing evidence-based care to housebound patients in their own homes. You will manage referrals, assess, plan, implement, and evaluate individualised patient care, promoting self-care. The position involves working closely with a friendly, supportive team and collaborating with a multidisciplinary team including Therapy Teams, General Practice, and Specialist Services. You will also provide advice and support to junior staff and assist the District Nurse/Senior Community Nurse in managing the healthcare environment.
