Role summary
This role is for a compassionate and motivated Community Assistant Practitioner to join a therapy team. You will support patients in their homes, care homes, and community settings, assisting with rehabilitation programmes and palliative care. The aim is to help patients regain independence, achieve personal goals, improve quality of life, and reduce hospital admissions. You will work as part of an integrated multidisciplinary team.
Essential criteria
- NVQ 3 or equivalent knowledge and experience.
- Foundation degree programme, or equivalent demonstrable knowledge and skills acquired through training and experience.
- Good general education.
- Experience of working in a health or social care setting.
- Experience of working with people, in the statutory / voluntary / private sector.
- Experience of working with people at End of Life.
- Literacy skills.
- Evidence of ongoing learning.
Desirable criteria
- Experience of working in people's homes.
- Skills, Abilities and Knowledge
- Knowledge of additional specialist areas through in-house training & short courses.
- Good communication skills.
- Highly motivated.
- Knowledge of multidisciplinary team roles.

Job summary
We are looking for a compassionate and motivated Community Assistant Practitioner to join our therapy team, supporting patients in their own homes, care homes, and community settings. Working as part of an integrated multidisciplinary team, you will assist in delivering rehabilitation programmes, support individuals living with long-term conditions, and contribute to palliative care interventions. Your role will focus on helping patients regain and maintain independence, achieve their personal goals, improve their quality of life, and reduce the need for unnecessary hospital admissions. Previous therapy experience is desirable.
Main duties of the job
- To contribute in the delivery of high standards of health care to patients within their own homes, care homes, the travelling community, Community Hospitals, and clinics, by assessing, planning, implementing and evaluating packages of care.
- To work within a reablement philosophy, using strengths-based assessments and actively promoting independence for the patients.
