Role summary

This role is for a Band 7 Community Nurse Team Lead, covering a maternity leave, within the District Nursing team at Carlisle & District ICC. The successful applicant will provide clinical leadership and operational management to the community nurse team, manage the clinical caseload, and support patients with complex needs in the community. The role requires an SPQ qualification or equivalent and focuses on delivering high-quality care within an Integrated Care Community model.

Key responsibilities

Essential criteria

  • Current NMC registration 1st level registration
  • BSc/MSc/SPQ equivalent clinical nursing experience or advanced practise
  • Evidence of recent Continual Professional Development
  • Significant post registration experience
  • Leadership or Team Management experience
  • Experience of service modernisation or change management
  • Demonstrate clinical knowledge in acute and chronic disease management
  • Knowledge of good clinical practice
  • Knowledge of current health and social care policies and legislation
  • Demonstrable knowledge of Clinical Governance
  • Demonstrate clinical competence in minor illness management
  • Able to demonstrate excellent motivation and communication skills

Main duties of the job

This role requires you to demonstrate the skills and knowledge to support the provision of high quality care, to support patients with complex needs within the community setting. The role will also require you to provide clinical leadership and operational management of the community nurse team along with the management of the clinical caseload for the team.

ICCs aim to address the challenges of an aging population and increasing demand on health and care services by providing more out of hospital care and empowering people to take control of their health and wellbeing. This will require new ways of working to focus on preventing ill health and ensure services are sustainable for the future. These new ways of working will follow the same approach across all eight IC
