Role summary

This role is for an experienced Senior Nurse to join the Acute Care at Home service, a partnership between multiple NHS Trusts. The successful applicant will provide clinical assessment, monitoring, and support as part of urgent care pathways. They will deliver direct patient care, incorporate non-medical prescribing, and formulate tailored treatment plans in a community setting. The role also involves acting as a shift lead, providing clinical leadership, and driving service development.

Key responsibilities

  • Deliver direct care to a wide range of patients and incorporate independent non-medical prescribing, physical health assessment, diagnostic testing and interpretation.
  • Formulate tailored treatment plans in conjunction with the Ward Consultant.
  • Act as the shift lead working closely with the Matrons and nurse coordinators.
  • Be a professional role model to provide clinical leadership, support and guidance to the Virtual Ward team and ensuring full clinical oversight of quality and safety.
  • Initiate, manage and drive change in practice for the benefit of patient care and service development in line with local and national policy and the NHS People Plan.
  • Hold delegated managerial responsibilities under the supervision of the Matrons and provide ongoing support to junior members of the team, participating in induction and training programmes.

The Acute Care at Home service is a partnership between the University Hospital of North Midlands (UHNM), University Hospital of Derby and Burton (UHDB), Royal Wolverhampton Trust (RWT) and Midlands Partnership Foundation Trust (MPFT). The successful applicants will join an integrated, collaboratively focused team, employing proactive clinical practice and knowledge, toprovide clinical assessment, monitoring and support, as part of our urgent carepathways.

The post holder is required to hold a non-medical/supplementary prescribing qualification. However, applications will also be considered from clinically experienced applicants with the relevant skillset currently undertaking the course or willing to undertake on appointment within the first 12 months.
