Role summary
This is an exciting opportunity for a Community Staff Nurse to join innovative community nursing teams in East Cheshire. The role involves delivering high standards of effective nursing care, holistically assessing patient needs, and developing and evaluating care plans. You will work autonomously and as part of a multi-disciplinary team, supporting patients with long-term conditions and the elderly population. Travel is an essential part of this role.
Essential criteria
- Registered Nurse to degree / diploma level
- Current registration with NMC (Nursing and Midwifery Council)
- Evidence of recent professional development in an up-to-date portfolio
Desirable criteria
- Non-medical prescriber
- Teaching / Assessing qualification
- Knowledge & Skills
- Ability to assess clinical situations and act appropriately
- Able to assess, plan and evaluate patient care taking into account the needs and desires of the patients
- Awareness of effects of ill health and the ageing process
- Awareness of the principles and practice of SSKIN (Surface, Skin inspection, Keep moving, Incontinence, Nutrition)
- Computer literacy including Knowledge of Microsoft office systems and use of email
- Leg ulcer assessment and management, Experience in palliative care, end of life care.
- Understanding of Gold Standard Framework
- Diabetes care/management.
- Understanding and/or experience of managing patients with (LTC Long Term Care) needs
- Recent previous experience within a comparable role / transferrable skills
- Demonstrable knowledge of assessment and therapeutic interventions in area of practice
- Experience of developing programmes of care for an individual or groups of patients/clients and of providing advice
- Ability to prioritise and organise workload effectively
- Experience of using electronic patient/service user record systems
- Ability to communicate in a professional way with people at all levels of understanding either face to face, on the telephone, e-mail or written communication
- Compassionate - with exceptional interpersonal skills with the ability to communicate effectively with staff, patients, carers and relatives (as appropriate) remaining sensitive and empathetic
- Experience of mentoring students / supervise other junior staff
- Experience of working within a multi-disciplinary team
- Previous experience delivering care in a community setting
- Experience of working in a rehabilitation environment
- Experience of working in multi-agency services
- Experience of mentoring
- Knowledge and experience relevant to area of practice
- Understanding and / or experience of interagency working

- Must be able to undertake all aspects of the role whilst working as a lone worker
- Able to organise own workload and deal with unpredictable nature of the role
- Ability to travel across the county with heavy / bulky equipment and the ability to travel from patient to patient, whilst maintaining a consistent caseload

Job summary
An exciting opportunity has arisen for a community staff nurse at 37.5 hours per week, available to join our innovative community nursing teams across our neighbourhood services including the Out of Hours Neighbourhood Service.
The Community Nursing Teams of East Cheshire are leading the way in integrated working. Closer working relationships with colleagues in Therapies and Social Services have been formed, as part of working aligned to Care Communities with new programmes and projects being developed with other specialties, supporting multi-professional working.
Being part of our friendly and supportive teams is an opportunity to take forward the expanding role of community nursing within the multi-disciplinary neighbourhood teams to meet the long term condition agenda and the needs of our elderly population. Our dynamic management team are supportive and open to ideas and innovation.
