Role summary
This role is for a motivated Band 5 Community Nurse to join the Mid Bedfordshire Primary Home Care Team, delivering high-quality nursing to patients in their homes. You will assess, plan, deliver, and evaluate nursing care for patients with diverse health needs, working within a multidisciplinary team. The position focuses on promoting independence, preventing hospital admissions, and supporting patients with complex needs. It offers opportunities for skill development, involvement in self-care facilitation, health promotion, and innovative projects.
Key responsibilities
- Assess, plan, deliver and evaluate nursing care for patients with a wide range of health needs in the community.
- Work as part of a multidisciplinary team to promote independence, prevent unnecessary hospital admissions, and support patients to receive high-quality care in their own homes.
- Deliver responsive high quality nursing to patients living in Sandy, Potton, Gamlingay and surrounding villages within Mid Beds.
- Be involved in facilitating self-care for patients with long-term conditions, health promotion and innovative activities and projects.
- Work a 7.5 hour shift pattern covering 7 days a week from 08:00-21:00.
Essential criteria
- Registered General Nurse
- Demonstrate evidence-based practice and up-to-date clinical skills.
- Ability to mobilise and stands for most of shift; and kneels and crouches to deliver clinical care e.g. dress wounds.
- Ability to concentrate on providing clinical care such as drug calculations, carrying out tests and managing interruptions and unpredictable patient/client /carer behavior
- Occasional distressing or emotional circumstances such as imparting unwelcome news, care of terminally ill or safeguarding issues
- Post holder could work in occasional unpleasant conditions during home visits and could come across unpleasant body odours, dust, noise/ Body fluid or even verbal aggressions
- Evidence of and commitment to multi- oprofessional working.
- Understanding of the interface between primary and secondary care
- oComprehensive understanding of the roles of others in health and social care
- Literate in IT/Computer Skills
- Evidence of knowledge of clinical governance
- Knowledge of the principles of Safeguarding of Vulnerable Adults and Children.
- Knowledge of the principles of health promotion and prevention of illness.
- Good oral communication skills based on fluency in English language
- Work within clearly defined clinical and professional policies
- Ability to work independently seeking appropriate support and advice as necessary
- Works within codes of practice and professional guidelines
- Current UK driving license and access to a car
- Ability to travel across sites and across Trust boundaries to meet patient needs and attend meetings, etc.
Desirable criteria
- Evidence of post-basic professional development
- Post-registration qualification in a relevant clinical field
- Relevant teaching/mentorship qualification or ocommitment to undertake
- Post-registration nursing experience, including community experience
- Be familiar with SystmOne in order to use the single assessment documentation in achieving person centred care.
- Be a proactive nurse with a strong desire to lead on the delivery of quality, holistic care
- oDemonstrate an understanding of national government initiatives in health care.

We are seeking a motivated, dynamic, and passionate Band 5 Community Nurse to work within our Mid Bedfordshire Primary Home Care Team, the team deliver responsive high quality nursing to patients living in Sandy, Potton, Gamlingay and surrounding villages within Mid Beds. We are passionate about patient centred care, support our nurses to lead and deliver excellent and compassionate nursing and value our clinical teams as the real heart of the NHS. You will work closely with other members of the MDT to deliver care to patients with complex needs.
