Role summary
This role is for a Community Healthcare Support Worker within the Older Persons Mental Health team in Bordon and Petersfield. You will provide care coordination and recovery-focused support to individuals with organic and functional mental health needs, including post-diagnostic support for dementia. The position involves home visits and working as part of a multi-disciplinary team to improve patient outcomes. It is a part-time role, 22.5 hours per week.
Key responsibilities
- Work as a core member of the Memory Assessment Services and support functional patients in the community.
- Provide post-diagnostic support for patients with dementia or functional illness.
- Assist with treatment initiation, medication titration, care planning, and risk management.
- Signpost service users to other professional and voluntary services.
- Work under the direction of qualified staff to promote recovery-focused goals.
Essential criteria
- *Literacy and numeracy skills
- *Full driving licence and use of a vehicle
- *Experience of working with older persons both organic & functional preferably in a community setting
- *Understanding of organic and functional illness.
Desirable criteria
- *Completion of NVQ Level 3 in Health and Social Care

Job summary
The Bordon and Petersfield Community Older Persons Mental Health (OPMH) Team operates from the main team base at Chase Hospital and covers Petersfield, Bordon and some of Waterlooville working 9-5 Monday -- Friday. The team is approachable and open. We support each other and have developed a culture of honesty and integrity.
Please note this role is part time, 22.5 hours a week.
This is a lively, dynamic and busy multi-disciplinary team which provides care coordination and recovery focused care for individuals with both organic/functional mental health needs. This is an opportunity to demonstrate your skills as a Healthcare Support Worker, who is focused on continually improving outcomes for patients.
You will play an important role in delivering excellent care for our patients and working as part of the team to provide the right support and assistance to our patients in their recovery. You would be joining a supportive team that work together closely.
Due to the nature of this post, you will need to carry out home visits and attend patient meetings therefore, a car driver is essential with access to own car.
